What Are Structured Observations?
Structured Observations are customizable report forms designed to capture consistent and detailed information. For example, you can create a Monthly Lighting Inspection Report with specific fields that officers must complete.
Available field types include:
Date picker
Time picker
Dropdown selections
Yes/No options
Short and long text fields
Number inputs
Media uploads
Signature fields
Create a Custom Form
Select your name or profile image in the bottom-left corner of the web portal.
Select Account settings.
Select Custom Forms.
Select Create form.
Enter a Form name.
Optionally, enter a description that explains when officers should use the form.
Under Availability, choose whether the form should be active
Choose where the form should be available:
Select Available at all active sites to make the form available across your account.
Leave this option unselected to assign the form to specific Sites.
If assigning the form to specific Sites, use the search bar under Assigned sites to find and select each Site.
Select Create form.
Note: Creating the form does not add any questions or fields yet. Open the form after creating it to build its field sequence.
Add Fields to a Custom Form
From the Custom Forms list, select the form you created.
Select Add field.
Enter a clear Field label. This is the prompt officers will see when completing the form.
Select a Field type.
Available field types include: Text, Long Text, Number, Yes/No, Dropdown, Multi-select, Date, Time, Photo/Video, Signature, Section Header, Divider.
Enable Required response if officers must complete the field before they can submit the observation.
Save the field.
Repeat these steps for each question, instruction, or structural element you want to include.
